It's the normal Japanese EP CD (+ DVD) named '天使のくれた奇跡' (The miracle given by an angel), also named 'Angel'

What makes this different is the presence of an additional mini-booklet composed of photos of libera taken during their visit to USJ Park in Osaka in the summer of 2015.
This mini-booklet is simply added to the outside of the normal CD, in a small transparent packaging sleeve.

This CD and its additional booklet could only be purchased in USJ Park, back when the park used the song 'Angel' as its theme song for the end-of-year holiday season, from 2015 to 2017.
